All of this. But take a peek at your medical records sometime, when your doctor is typing. If you are a woman, it describes your appearance, including, if you are wearing “appropriate” makeup and if you are “well-dressed”. And I generally am fairly ill when I go. (Knowing this, I always wear at least business casual).

I’ve never noticed this notation in my husband’s, but it’s always noted in mine (I’m a woman).

So, even doctors do this, to “well-dressed” female patients. I have no idea what this signifies to other medical professionals, but it seems…like a dog whistle for something else.
